The website intake is only a request for contact. It is not a telehealth visit and is not itself consent to medical treatment.

Clinical evaluation

If you proceed to care, the treating clinician or medical practice will provide any required telehealth disclosures and consent appropriate to your location before clinical services are delivered. The clinician is responsible for determining whether telehealth is appropriate and whether any medication or laboratory testing is medically indicated.

Limits of telehealth

Telehealth may not be appropriate for every condition. A clinician may recommend in-person care, additional testing or emergency evaluation when necessary.

Prescriptions

No prescription is guaranteed. Prescribing requires an individual evaluation and compliance with applicable federal and state law. Availability varies by patient, state, prescriber and dispensing pharmacy.
